Why? Per CMS requirements, in order for one of our agents to assist in reviewing Medicare plan options, we need to first obtain your formal request in the form below. When selecting which products you would like to discuss, if you are uncertain, please check all the boxes. If you omit one and later have questions about that one, we will have to obtain a new form and will need to reschedule the appointment.
